Hampshire County Council (HCC) is seeking a suitably qualified and experienced organisation to carry out person centered, strengths-based assessments and reviews, along with associated support planning and any additional assessment work arising under MCA or CHC, in accordance with Care Act 2014 principles and in line with HCC's policies. This work is to be completed either in person with the individual being assessed/reviewed or on a remote basis (video call).
