Young Persons Shared Service Basingstoke

A call off mini competition from the Hampshire Framework for Shared Living and Learning Disability Support. A new service to provide support for five young people with a learning disability leaving education and make the transition to live together in the community. To support the individuals to build social capital by leading active, healthy, socially responsible and socially inclusive lives as part of their local community. Providing a level of support which is appropriate to the needs of the individuals with a focus on independence and personalised outcomes and reduce dependency on support provided by paid carers.
