Overview
The London Borough of Hounslow and Hounslow CCG are to establish a Pilot project for reablement care which will commence mid-November 2017 for up 18 months. The first 12 months of the Pilot is to measure the service delivery output to seek an efficient approach to re-ablement care at an increased capacity. The remaining 6months of the Pilot is to re-design the service delivery model based on the evaluation of the Pilot performance, whilst at the same time tendering the service.
