The Patient Safety Incident Response Framework (PSIRF) is a key part of the NHS Patient Safety Strategy. It aims to significantly enhance the quality of learning and improvement following patient safety incidents. The work will seek to describe current understanding and practice in relation to the oversight of learning from patient safety incidents and align this with the oversight requirements outlined in PSIRF.
