NHS Trafford CCG and key partners have progressed integrated care in Trafford for several years and it remains the key strategic objective throughout Trafford's Health Economy, as set out in Trafford's Strategic Plan. Developing a new model of care for patients with more activity delivered in an out of hospital setting has to deliver improved patient pathways and provide an alternative to secondary care with greater integrated working between primary care, community services and the social care system, ensuring patients receive their care by the right person, in the right environment and at the right time.
