Support Services for People Affected by Stroke

The Service provides information, advice, and emotional support and helps the stroke survivor and their families to adjust to the changes caused by the stroke and optimising the quality of life for both the survivor and family members. Support workers establish and maintain regular contact with stroke survivors and their families, including telephone contact and home visits. Referrals to the Service are received primarily from care management teams, and prioritise people with a recent stoke being discharged from hospital but may also accept referrals from other sources including hospital staff, G.P.'s, other statutory health & social care services and self-referrals.
