Rapid Discharge Service Lot 7

The Domiciliary Care Rapid Discharge Service (RDS), known as the RDS through this document, is a short-term reablement and domiciliary service offering timely discharge of individuals who may have an eligible Care Act need. These individuals may need support with discharge from a Hospital setting to their own home or may require support to prevent the need for a hospital admission, thereby supporting flow and effective management within the wider system.
