Overview

The Domiciliary Care Rapid Discharge Service is a Covid-19 response short-term service put in place to support the timely discharge of service users who may have an eligible Care Act need or need support with discharge from a Hospital setting to their own home. The service will be used where a long-term package of care is likely to be needed. All service users discharged into this service will be followed up by a Hampshire County Council member of staff. Individuals discharged into this service may be Covid-19 positive and there is an expectation that the service provider will accept these individuals into this service.
