Overview
Home to Settle is a service being procured to support Winter Pressures in the East Kent area. Home to Settle - known more commonly as "Assisted Discharge" - is a service which identifies patients who are non-clinical blockers who can be supported in returning home earlier. The service also includes a front-door support which helps prevent re-admission of patients, particularly elderly people who live alone. Due to the nature of this Service, we are working on a very tight deadline, this opportunity is only open for the minimum required period, closing at 5pm on Wednesday 5 December.
