CNWL is looking to commission a bespoke High Intensity Use service across the North West London Boroughs of Kensington & Chelsea, Westminster and Hillingdon to support this cohort of individuals. The Trust would like to develop a different, non-clinical approach to address some of the underlying social needs that are not being met by the existing system. The Trust seeks to commission a Provider to adopt a social-led approach to support this cohort of patients who frequently present to A&E and/or are re-admitted into hospital within a short period of time, as well as those at risk of this.
