Overview
The development of person centred coordinated care is a key building block in Southampton’s Better Care Plan. The plan has refocused attention of the proactive or preventative agenda for health, care and wellbeing and provides the main driver for the commissioning of the Community and Wellbeing Team. This service will encompass those aged 18+ registered in Southampton City who have a specified need for proactive care, where this is not already provided by other core and specialist community services.
