Dementia Support Services

Buckinghamshire Council is seeking to commission an organisation that will act as a single point of contact to provide pre- and post-diagnostic advice and support for individuals and their carer's throughout the dementia journey. The service will empower people to make informed decisions about their care to maximise their quality of life, reduce the risk of crisis and admission to hospital and enable the individual to live independently in their communities for as long as possible.
