Overview

Community Stroke Services - Sandwell The Stroke Support Service must provide information, emotional support and practical help to the stroke client, their families, and carers for a period of 12 months post stroke through 6 weeks, 6 months and 12 months reviews. The Provider must work in the hospital and the community alongside a multidisciplinary team of professionals across health and social care to provide information and advocacy support for stroke clients and their families/carers as they work towards adjusting to the change in circumstances caused by stroke. The ICB intends to award this contract under Direct Award Process C. The approximate lifetime value of the contract is £86,994
