Overview
Doncaster Metropolitan Borough Council and NHS Doncaster Clinical Commissioning Group invite detailed proposals from experienced Providers to deliver a new care and support service in Doncaster, known as Commissioned Care and Support at Home. The Contract will commence on Monday 3rd October 2016 and will run for a period of 7 years with the opportunity to extend for a further 3 years (1 X 2 years and 1 X 1 year). The new service will deliver support to Individuals with varying assessed levels of needs, in order for them to live at home and within their community for as long as it is safe and appropriate to do so.
