CARETAKER PROVIDER Required: Due to the current contractor requesting to terminate their APMS contract in advance of their contract end date, NHS England wishes to engage a practice or organisation to “care take” the Chestnuts Park Surgery, Laurels Healthy Living Centre, 256 St Ann’s Road, London, N15 5AZ (the “Practice”). The caretaking contract will take effect from 9th March 2016 until 30th November 2016 (unless terminated earlier or extended by NHS England). NHS England reserves the right to extend the contract for a further period of up to 6 months at its discretion.
