Basic Description of Service Functions: The Provider will deliver identification of Carers not known to services to ensure they are being offered support and tracking outcomes from support offered; provision of advice, information and guidance for Carers; completion of statutory Carers Assessments and production of Support Plans where these are required (utilising a range of digital, telephone and face-to-face support channels including online self-assessment) and facilitating the development of support networks for Carers across Cumberland.
