Solihull CCG are inviting bids for a Care Navigator Service to support and enable people with frailty to find their own solutions to problems and help them to navigate and access relevant services, providing a cost effective link between people and their GP and other health and social care professionals. The service will improve people's well-being and independence and reduce unnecessary and inappropriate use of primary and secondary care services. The Contract period will be for 2 years with an option to extend for up to a further 2 years at the discretion of the CCG - so for clarity, the contract will start 01st April 2017 with the initial period ending 31st March 2019.
