Suffolk County Council is changing the way in which it commissions home care services. The new Home Care Model features four elements: • Responsive - urgent same day community response where any requirement for on-going home care and support is not yet determined or where a change of needs is being considered, this could be either a short term period of assessment or and offer when required a reablement intervention for people to maximise their independence and enable them to remain at, or return to, home.
