Hospital Discharge The Hospital Discharge service will support people aged 60 years+ who are being discharged from Hospital without a formal care package and without the assistance of friends or family. The aim of the scheme is to support people up to the first 3 weeks after discharge to prevent readmission to hospital. The package offered by the Hospital Discharge Service is designed to ensure older people can remain independent in their own home, instead of taking away the skills required for independent living the team are proactive in encouraging and supporting those skills are maintained.
