The Adults' Outreach Service is commissioned to enable people with assessed needs to lead a full life, maximising independence, promoting health and wellbeing and supporting people to remain safe and connected to local communities. The service is designed to support needs, and those with more complex needs, in an innovative way. The Provider will deliver the Service to adults aged 18 and over who have an eligible Care Act 2014 assessed need, identified by the Council’s Assessment Teams.
