Waltham Forest Anticoagulation Services

Waltham Forest CCG is seeking to commission a primary care-led service model for the monitoring and management of anticoagulation patients. The service is required to be delivered from a minimum of five sites across the borough, with at least three of these sites being local pharmacies.
