North Lincolnshire Council requires a Stroke Recovery, Communication Support and Stroke Prevention Services for North Lincolnshire residents who are at risk of stroke, and stroke / TIA (transient ischaemic attack) survivors and their carers. The aim of the service is to provide an innovative person-centred prevention and support service to individuals at risk of, or who have a diagnosed stroke or hospital admission for a TIA. The client base is varied and will include vulnerable people and people with complex needs including co-morbidities.
