This is a Voluntary ex Ante Transparency Notice in Pursuant of Regulation 99 of the Public Contract Regulations (2015) The aim of the pilot scheme is to develop new pathways for elderly people who have a high dependency on formal care and are at risk of hospital admission. The scheme relies on significantly enhancing the role of the voluntary sector, to focus interventions on what the individual, their carer, their family and the community can do for themselves.
