The Interim Live in Care Service is a short-term service, aimed at providing an effective, flexible and responsive approach to discharge from the acute and community hospitals and their surrounding hospital systems for individuals who are medically stable and may require further assessment of their needs. The service will be used where a long-term package of care is likely to be needed. All service users discharged into this service will be followed up by a Hampshire County Council member of staff.
