In line with the National Dementia Strategy, this service is focused on the quality of care for people living with dementia and their families through work on diagnosis. The Service provides 1 to 1 support for people at an earlier stage of dementia. Delivery of Dementia PDS is currently subject to a Scottish Government Local Delivery Plan (LDP), standard to monitor post diagnostic support delivery - People newly diagnosed with dementia will be offered a minimum of one year's post-diagnostic support, coordinated by a named link worker.
