Durham Rehab Community Activities

The service will be an integrated referral system delivered alongside Durham rehab service supporting service users through community rehab who have been discharged. The service will provide both early intervention services and prevention of readmission. The service will be provided by 2 x 0.8 WTE Mental health wellbeing recovery practitioners and 1 x Activity coordinator to provide a coordinated programme of support and fill resource gaps and provide support to the Rehabilitation team within TEVWS Durham sites.
