The Council wish to commission a new NHS Health Check Service: that utilises both Primary Care and the community infrastructure with the aim of improving the health and wellbeing of eligible adults; through the promotion of early awareness, assessment and management of the major risk factors for Cardiovascular Disease (CVD).
