East and North Herts CCG are looking to commission a structured education programme for newly diagnosed patients with type 2 diabetes (and their carers) to ensure they understand the condition and how they can self-manage their condition. Diabetes education can help improve glycaemic control, self-care and emotional well-being and should be offered to every person and/or their carer at and around the time of diagnosis.
