Development of an implementation plan for the Integration Index

The Integration Index is a Long-Term Plan commitment to introduce a new nationally-coordinated data collection of the experience of integrated care, targeted at specific user groups and their carers. It will ask the different audiences a short set of meaningful questions in relation to care and support that span a number of different health and care services.
