Dementia Resource Community (DCR), have been commissioned since October2024, to reduce A&E attendance and inpatient bed use, by providing three memory assessment nurses. Additionally, DRC undertake Diadem assessments in care homes thus releasing capacity to the Older Adult CMHT. DRC cover all geographical areas. Chester patch is heavily supported as there are no step-down services in this area and also for individuals who do not need specialist input as they are referred to DRC.
