Community Navigator & Well-Being Service

PCC intends to procure a Community Navigator and Well-being service, which aims to maintain and improve the health, wellbeing and independence of eligible adults in Peterborough. The service delivers an early intervention and prevention approach that focuses on people and their community's capacity to self-care and manage as the first option. The objective is to reduce the risk or deterioration of the person and subsequent increased reliance on statutory services.
