Community Based Falls Prevention Service

A Single Point of Access for a community based falls prevention service with initial assessment and therapy from clinicians, and equipment ordering to meet low level needs & signposting on to other services. Documentation of need and intervention result into a database, which will be accessible by other services across the Council and NHS and used for individual and themed pro-active falls prevention work across the whole falls prevention pathway.
